  1. Scope of Service: Teeth Retain Design is a custom dental appliance manufacturing service. We specialize in creating retainers and other dental devices based on the specifications and impressions provided by the customer. 

  1. Not a Medical Provider  
     I understand that Teeth Retain Design is not a dental/orthodontic office and does not provide dental or medical advice, diagnosis, or treatment. 

  1. Impression Accuracy 
     I confirm that I will follow the impression-taking instructions carefully. I understand that the fit and effectiveness of the appliance depend on the accuracy of the impression I provide. If my impression is distorted or inaccurate, I may need to submit a new impression at my own cost. 

  1. Product Use and Care 
     I agree to use the appliance only as directed. I understand that improper use, cleaning, or storage may result in damage to the appliance or discomfort. 

  1. No Guarantee of Results 
     I understand that while the appliance may help maintain alignment after orthodontic treatment, individual results may vary. Teeth Retain Design makes no guarantee regarding long-term dental outcomes or alignment retention. 

  1. Assumption of Risk and Liability Waiver 
    By signing below, I voluntarily assume all risks associated with the use of the product. I understand and agree that Teeth Retain Design is not liable for any movement, shifting, or alteration of my teeth resulting from the use of its dental appliances.
